The scene : The galleries of the wastewater treatment plant serve as tunnels for Jamie when she flees Smith's Grove, and reappear as the basements of the sanatorium, briefly in the finale and more extensively in the Producer's Cut.
Cottonwood Paper Mill
Cottonwood Heights, United States
Mill built in 1883 at the entrance of Big Cottonwood Canyon, operated as a paper mill until a fire in 1893. Partially reconstructed in 1927, it became the dance hall of the Old Mill Club, then hosted concerts, a haunted house, and a craft shop before being condemned by the city in 2005. The place also appears in SLC Punk! (1998).
The scene : The old paper mill focuses a good part of the film: the room with stone walls where Jamie's baby is born, the entrance of Dr. Loomis's house, the door and entrance of the staff at Smith's Grove, and the room where the Thorn cult performs its ritual.

Salt Lake City, United States
Building from 1932, originally a hospital of the Veterans Administration then an annex of the Church of Jesus Christ of Latter-day Saints, transformed into condominiums in 2005. Its rear facade has been completely remodeled and a second brick building that faced it has disappeared.

Former Queen of Angels Hospital, now occupied by the Dream Center.
The scene : During the summer 1995 shoot, the old hospital serves as the operating rooms and mesh corridors of Smith's Grove for the film version, as well as part of the final confrontation between Wynn and Loomis; Tommy, Kara, and Danny leave in a car.
Los Angeles, United States
The scene : On this set in downtown Los Angeles, the production recreated the barn where The Shape impales Jamie on a rototiller, as well as part of the Strode's basement, for the deaths of Jamie and John Strode from the theatrical version.

Pierre Lassonde House
Salt Lake City, United States
Former building no. 604 of Fort Douglas, transformed in 2008 into executive offices and renamed Pierre Lassonde House; the renovations have made it largely unrecognizable. It also appears in SLC Punk! (1998).

1926 Lincoln Street (Salt Lake City)
Salt Lake City, United States
The scene : New Myers house from the saga, now inhabited by the Strode family: John takes down the sign bearing the name Myers, Kara finds it locked upon returning and walks around it, then, on Halloween night, she flees Michael with Danny before John meets his demise in the basement.

The scene : Danny walks up Lincoln Street; Barry Simms' van passes in front of this house and then turns onto Ramona Avenue, revealing the man in black.
